When someone says I was worried about you it simply means she was stressed and unhappy thinking about you, thinking something bad must have happened to you. Thinking you must be in some problem or something must have gone wrong

4 Incident
It is an event or occurrence that has been taken place in past mostly unexpected and undesirable
They are mostly bad or sad event that have taken place
You can use it this way
1. Don't remind me of that incident.
